3602 - Medium chain acyl-coa dehydrogenase deficiency (MCADD)
What is the appropriate code for Medium chain acyl-coa dehydrogenase deficiency (MCADD)?
MCADD is a rare genetic condition where a person has problems breaking down fat to use as an energy source. This means that someone with MCADD can become very ill if their body's energy demands exceed their energy intake, such as during infections or vomiting illnesses when they're unable to eat.
The best fit code I can find is:

While trying to follow the advice in VICC Query #3627 ACS 0010 Clinical documentation and general abstraction guidelines documented descriptions and ACS 0010 Clinical documentation and general abstraction guidelines, we have determined that our coders have been interpreting the advice differently.
Can you please provide advice on which code should be assigned for documentation of “(down arrow) BP” (assuming that it meets ACS 0002)?

2535 - Abdominoplasty for reduction of the excess skin - retired 30/6/2015
As a positive result of lap band surgery many patients represent for abdominoplasty for reduction of the excess skin.
After checking the 10 AM Commandments Volume 8 Number 2 September 2001, I refer to Abdominal Apron/overhang for the use of Code E65 Localised adiposity.
Whilst the wording of excess/loose skin or marked weight loss requiring abdominoplasty is not the same as 'abdominal apron' or 'abdominal overhang' can this code be applied for ...

3811 - Abnormal blood pressure readings
We are seeking clarification on the advice in VICC 3627 ACS 0010 Clinical documentation and general abstraction guidelines documented descriptions and how to code documentation of (down arrow) BP or (arrow up) BP. We note that test results are required to verify that a BP reading is abnormal, i.e. high or low, but are unsure if documentation on an observation chart is sufficient to verify BP readings are abnormal.
Our observation chart has colour coded sections with ins...

3406 - Abnormal INR secondary to antibiotics with warfarin use
High INR is clearly documented as being secondary to antibiotics. Warfarin is adjusted accordingly during patient's admission. I have also discussed this with the clinician to support this documentation.
When I code R79.83 Abnormal coagulation profile as an adverse effect of antibiotics and Z92.1 Personal history of long term (current) use of anticoagulants, I have an error appear on the Codefinder referring to ACS0303 - do not assign Z92.1 and R79.83 in the same episod...
