3486 - Bifocal germinoma
A bifocal germinoma is a tumour that arises in the suprasellar and pineal regions of the brain at the same time.
Would it be correct to code them as two separate primary lesions: suprasellar to C71.9 Malignant neoplasm of brain, unspecified and pineal gland to C75.3 Malignant neoplasm of pineal gland?...

2293 - Bile leakage following laparoscopic cholecystectomy - retired 30/6/2013
Victorian ICD Coding Newsletter February 2004 #1873 indicates to code bile leakage following laparoscopic cholecystectomy as it is a DHS performance indicator (CI-4).
In this particular query, there is no indication that the bile leakage was treated, monitored (to meet ACS 0002). Do we follow this advice because it is an indicator or does there have to be evidence of treatment ?

3731 - Biliary colic
Principal diagnosis: Biliary colic
Operations/ Procedure: Laparoscopic cholecystectomy
1.Can VICC please provide advice on what is the correct code to assign when there is documentation of biliary colic?
When discussed with one of our surgeons, it appears that it is a term used to describe symptomatic cholelithiasis without cholecystitis and is not the same as choledocholithiasis.
The index leads us to code ca...

3883 - Biodegradable temporising matrix (BTM) wound repair
Patient was admitted for treatment of an infected pre-tibial abscess. One of his procedures was a debridement, BTM and VAC application.
In regards to 'BTM', the operation report simply says "BTM inset with 4-0 VR". This was performed after debridement and before application of the VAC dressing.
Google search advises:
"BTM (biodegradable temporising matrix) is a synthetic, biodegradable and biocompatible device designed to facilitate the dermis to ...

2099 - Biopsy male perineum - retired 30/6/2010
I have a general problem with procedure codes for those to do with the perineum, both indexing inadequacies & code title, etc problems. All procedures appear in the gynaecological section but can be performed on males as well.
My problem this time is a male patient who had an biopsy of a perineal recurrence of Ca Rectum - there is no look up in the index for any such procedures for the perineum, and as previously mentioned, the word perineum appears in the Block titles (vulva & perin...

3876 - Biopsy of Ampulla with endoscopic ultrasound (EUS)
Can VICC please confirm the correct procedure code(s) to assign for a patient who has an EUS performed with a biopsy of ampulla?
IHPA has stated that a separate code for biopsy of pancreas should be assigned when performed under endoscopic guidance. There is no clinical code for biopsy of ampulla however from our understanding the ampulla is where the pancreatic and bile duct connects.
We would like to know whether the biopsy is reflected within ...

3798 - Birth episode for neonates
Can VICC please confirm whether the advice in retired VICC query #2042 Conditions arising in the perinatal period and retired VICC query #2411 Conditions in neonates is still valid?
Should codes for neonates and interventions related to the sick neonate be assigned for babies admitted to our facility at > 28 days of age if the baby has remained in hospital since their birth episode (i.e. the baby has not been discharged home)?...
