Administration of surfactant to newborn
Publication Date: March 2005
ICD 10 AM Edition: Fourth edition
Retired Date: 30/6/2010
Query Number: 2028
I wish to clarify if administration of surfactant to premature babies should be coded, and if so, what code should be used in 4th edition?
Search Details: I have searched the NCCH query database and note that I have seen query 444. The advise is to assign 92048-00 [1849] 'Other lavage of bronchus and trachea', however this query dates back to 1998 and this code no longer exists. When looking up lavage of trachea, via intubation in the index, it directs the coder to use 22007-01 [568]'Management of endotracheal tube, single lumen'. This code does not seem appropriate to use for this procedure. I have also looked at block 1889, but there does not seem to be an appropriate code to use here either.
Surfactant can be given as prophylactic treatment for babies at a high risk of developing RDS or as therapeutic treatment of babies that have been clinically confirmed as having RDS. Survanta is administered via a gavage tube cut so that the tip lies 1cm above the end of the endotracheal tube.

Response
The NCCH supports the Victorian ICD Coding Committee’s assertion that 'administration of surfactant' should not be coded, as it is routine treatment for premature babies.
NCCH Query Q444 will be updated to incorporate this current decision.
The NCCH will consider the addition of this information to the relevant Australian Coding Standards for a future edition of ICD-10-AM.
