What is the correct code to assign for a patient admitted with septic shower? Currently there is no Index entry for Septic shower. The term Septic leads us to - "see also condition".

According to the internet septic shower is defined as: A septic shower is the sudden systemic influx of pathogens that have colonized in an inserted device triggered by the infusion of fluids into the device causing septic shock in the patient. This is a life threatening condition and requires urgent medical attention.

With the above definition are we able to code sepsis (A40.- and/or A41.-) with R57.2 Septic shock?

If a patient has a device are we able to assume the sepsis is due to the device and therefore we assign the appropriate T code with sepsis and septic shock?

VICC referred this query to the ACCD who provided the following advice:

Septic shower:
• is the sudden systemic influx of colonised pathogens
• most commonly associated with an in situ/implanted device
• often follows infusion of fluids into an in situ/implanted device
• may result in septic shock.

Where septic shower is documented as associated with the presence of a device, assign an appropriate code from T82-T85 by following the Alphabetic Index at Sepsis/due to/device, implant or graft/by type of device.
Assign codes for sepsis and septic shock as per the guidelines in ACS 0110 SIRS, sepsis, severe sepsis and septic shock. Assign external cause and place of occurrence codes, as applicable.
Amendments will be considered for a future edition of ICD-10-AM.

Further to the ACCD advice, VICC advises to follow the guidelines in ACS 0110 SIRS, sepsis, severe sepsis and septic shock to assign a code for sepsis when septic shower is documented. A additional code for septic shock can be assigned only when septic shock is documented in the record. If it is unclear whether the patient has septic shock then a clinician query can be sent for clarification.
