3858 - Admission note in emergency department (ED) time
Admission notes may be documented in the medical record while a patient is in ED awaiting formal admission to the ward. These often contain plans of care and thorough medical histories.
ACS 0010 Clinical documentation and general abstraction guidelines states that emergency notes do not form part of the current episode of care.
If the admission note is written in ED time, should this be treated as emergency documentation, or as documentation from...

3109 - Admissions for high risk neutropenia / neutropenic monitoring
At our hospital there has recently seen an increase in admissions for what is documented on the discharge summary as 'high risk neutropenia'. ¯We have seen similar admissions at other hospitals documented 'neutropenic monitoring'.
These are cancer patients who have had chemotherapy in a previous admission. They are admitted three to five days later because doctors consider them at high risk of developing neutropenia. Some do develop neutropenia during the admission a...

2089 - Adverse effect of IV methylprednisolone - retired 30/6/2010
Patient is given IV Methylprednisolone as an inpatient, and develops hyperglycaemia as an adverse effect. When looking up an adverse effect in
therapeutic use, the books direct you to use Y56.0 Local antifungal, anti-infective and anti-inflammatory drugs.
This code is not correct as the methyprednisolone was not used topically, but given as an IV.
We feel that Y42.0 Glucocorticoids and synthetic analogues would be more appropriate. ...

2777 - Adverse effects of clozapine
When coding adverse effects of clozapine the index leads to Y47.1 Benzodiazepines causing adverse effects in therapeutic use. Clozapine is an anti-psychotic used for treatment resistant schizophrenia and can have serious side effects such as myocarditis.
Advice from our Psychiatrist is that this should be classified as an anti-psychotic not a benzodiazepine and that Y49.5 Other antipsychotics and neuroleptics would be more appropriate. The community pharmacist also agre...

2162 - AICD / Pacemaker Insertions - retired 30/6/2010
Doctors at our hospital have advised that all AICD units have both pacemaker and defibrillator functionality. One lead from the AICD unit performs both functions (pacing and defibrillating). This lead is placed into the right ventricle.
Depending on the pacemaker function required depends on the number of leads to be inserted.
The problem that coders face is how should we capture the fact that one of the leads doubles up to act as a defibrillat...

2024 - Albumex infusion - retired 30/6/2010
I have two questions from two records which I coded on the same day for which I would need clarification please.
1. A baby admitted at birth, who had several problems, and received a transfusion of Albumex. MIMS lists Albumex as a 'plasma volume expander'. I followed the index entry in Vol 4 - Transfusion, blood, expander 92063-00 [1893] However, ACS 1615 (Specific interventions for the Sick Neonate) states to code this when performed for the neonate to 92062-00 Transfu...
