3467 - Anaesthetic coding
Can you please clarify if the previous advice regarding coding of general anaesthesia versus sedation is still correct? The advice we refer to is VICC query #2492 ACS 0031 Anaesthesia 2009 -2010 December Database retired 30/06/2015.
We would like to clarify does there need to be documentation of the use of an artificial airway such as endotracheal tube, laryngeal mask or Guedel (oropharyngeal) airway to support the assignment of a general anaesthetic (GA) code when only ...

2043 - Anaphylactic reaction to food - retired 30/6/2010
A patient with a known allergy to peanuts presented with anaphylaxis after eating food in a restaurant which he did not know contained peanuts.
Search Details: Previous advice in Coding Matters vol.4 no.1 in July 1997 and NCCH query #732 on 22.12.1998 both advised to code this to T78.0 Anaphylactic shock due to adverse food reaction and Y57.9 Other and unspecified drugs and medicaments, unspecified (causing adverse effects in therapeutic use).

3202 - Anaphylactic reaction/shock due to overdose/wrong substance
To assign a code for anaphylactic shock or reaction due to overdose, we
follow the index entry of:
Shock, anaphylactic
- Drug or medicinal substance
- - overdose or wrong substance given or taken T50.9 Poisoning by other and unspecified drugs, medicaments, and biological substances.
This leaves us with a T50.9 code without mention of the anaphylaxis. In contrast, if assigning a code for anaphylactic shock or reaction due to...

Patient admitted with and treated for angina. Ischaemic heart disease (IHD) noted on the history sheet. Is the angina the result/ outcome of the IHD, and therefore, IHD need not be coded, or should the IHD be coded as an additional diagnosis?...

3543 - Angioectasia - retired 31/5/2023
Could VICC please advise the correct diagnosis code to assign for documentation of angioectasia?
We have seen documentation of angioectasia of the duodenum or jejunum treated with argon plasma coagulation. Research online indicates gastrointestinal angioectasia are the most common gastrointestinal tract vascular anomaly and are characterized by ectatic, dilated, thin-walled vessels in the submucosa or mucosa. Angiectoasias are often located in the colon, and are less of...

3577 - Ankle syndesmosis injury and Maisonneuve fracture
This year we have seen an increasing number of footballers admitted to our hospital with a diagnosis of ankle syndesmosis injury. Some of the patients were treated conservatively and some of the others required surgical fixation to treat the injury.
We have also had a patient with the diagnosis of Maisonneuve fracture and he underwent an ORIF of ankle syndesmosis.
Our research indicates that an ankle syndesmosis injury is a sprain of the syndesm...

3323 - Annuloplasty with band
Patient admitted for a 35 mm taylor band annuloplasty of the mitral valve. Our interpretation of research is annuloplasty with band is a similar concept to annuloplasty with ring. The ACHI Index does not appear to provide a pathway leading directly to a code to classify the procedure of annnuloplasty with band.
Index pathway considered to classify the procedure of band annuloplasty of mitral valve:
Annuloplasty / mitral valve / with insertion of ...

2076 - Antegrade cerebral perfusion - retired 30/6/2010
Need to code 'antegrade' cerebral perfusion with deep hypothermia circulatory arrest.
The title for procedure code 38577-00 (642) describes 'retrograde' perfusion. However, the index entry for same lists 'retrograde' as a non-essential modifier.
Would it be acceptable in the meantime to use this code for 'antegrade' cerebral perfusion?

2638 - Anterior chest wall resection - retired 30/6/2017
Patient admitted into hospital for anterior chest wall resection. The procedure included resection of Body of Sternum, ribs, soft tissue and cartilage. The defect was reconstructed using a GORTEX/ Methyl Methacrylate patch. How do we code this?
Currently, coding resection of the chest wall includes lobectomy or pneumonectomy of the lung, however, no lung resection was performed in this operation.
