ACS 0940 Ischaemic heart disease, section title Acute Myocardial Infarction (Classification) , pg 153. The 3rd paragraph of this section states “codes from category I21 should be assigned for an infarction in both the first hospital to which the patient is admitted for treatment and any other acute care facility to which the patient is transferred within 4 weeks (28 days) or less from onset of the infarction”. Patient admitted to ED at 10.00am. Patient intubated in ED at 11.00am and immediately commenced SIMV. At 1.00pm decision is made to send the patient to the Cath Lab/Radiology/Theatre where the patient undergoes a procedure/surgery (still ventilated). Could you please provide some guidance re the use of 'with haemorrhage' as per ACS 1103. The wording in the standard is very specific: 'upper GI bleeding', 'ulcer, erosions, varices'; how strictly is this wording to be adhered to?
1) Can PR bleeding be regarded in the same way as upper GI bleeding as per VICC query 2512?

2855 - ACS 1204 Plastic surgery - retired 30/6/2020
Is query #1955 ACS 1204 Plastic surgery still valid or did it retire in June 2010?
The reason I am asking is because in the standards book, page 143, example 2, in this example they have not added the Z41.1 as an additional code. Shouldn’t they have added the Z41.1 into example 2 if the above query was still valid?...

We often see documentation of plastic surgery for conditions that are not recognised by ICD-10-AM e.g. excess size breasts. 1955 - ACS 1204 Plastic surgery - retired 30/6/2010
According to ACS 1204 Plastic Surgery ‘When the reason for surgery …..is documented code this condition as the principal diagnosis even if the procedure performed is of a cosmetic nature’.
If a diagnosis of “Mammary Hypoplasia” is documented and an augmentation mammoplasty is performed (Surgeon has given a cosmetic CMBS code). I have been coding Mammary Hypoplasia (Q83.8 Other congenital malformations of breast) as the principal diagnosis.

ACS 1404 Admission for renal dialysis includes the section:
